Caregiver · HHA · CNA
Our frontline. You deliver compassionate, hands-on non-medical care in clients' homes across Rittenhouse Square, Center City, the Main Line, and the greater Philadelphia metro. This is relational work with the same families over time, not a new stranger every shift.
What you'll do
- Assist with daily living: bathing, dressing, grooming, mobility, and transfers
- Support meals, light housekeeping, medication reminders, and companionship
- Follow the RN-reviewed care plan and note changes in the client's condition
- Communicate with your coordinator and the family so nothing falls through the cracks
What we're looking for
- Experience as a caregiver, HHA, CNA, or in a comparable care setting (or a genuine heart for the work and a willingness to be trained)
- Reliable transportation across the service area
- Ability to pass PA criminal-history and required background clearances
- Warmth, patience, and dependability. Chemistry matters as much as competence.
Care Coordinator · Scheduler
The nerve center of the agency. You match caregivers to clients, build schedules that respect both, field calls, and keep families and caregivers informed. When something changes at 2 a.m., you are the calm voice that sorts it out.
What you'll do
- Own caregiver scheduling and client staffing across the service area
- Handle intake calls, family updates, and day-to-day coordination
- Cover part of the 24/7 on-call rotation with the coordination team
- Keep records accurate in our scheduling and EVV systems
What we're looking for
- Scheduling, staffing, or coordination experience, ideally in home care, healthcare, or a fast-moving service business
- Unflappable under pressure, organized, and genuinely good with people
- Comfortable with scheduling software and quick to learn new tools
- Based in or near Philadelphia; this is an on-site role at our Walnut Street office
Registered Nurse · Care Manager
You set the clinical standard. You conduct initial assessments, build and review care plans, supervise caregivers per PA regulation, and are the clinical point of contact families trust. This can start per diem and grow with the agency.
What you'll do
- Complete in-home assessments and develop individualized care plans
- Review and update plans, and supervise caregivers per 28 Pa. Code Chapter 611
- Train and validate caregiver competencies per § 611.55
- Serve as the clinical liaison to families, physicians, and discharge planners
What we're looking for
- Active, unrestricted Pennsylvania RN license in good standing
- Home care, home health, geriatric, or community nursing experience preferred
- Strong assessment skills and clear, compassionate communication
- Comfortable working independently across the Philadelphia metro
